Can someone please explain to me why 1% of the population makes 96% of the total income in the world?

If what my school textbook tells me is true- then this system is really unfair! Democracy puts power in the hands of the few? I thought all men were created equal!...I guess I'll have to rethink that...What is the reasoning behind this system? Can anyone possibly expain it so I understand?
